A Healthier Cherokee County, One Step at a Time

Every few years, Cherokee County takes a close look at the health and wellbeing of our community. Through research, community input, and local data, we identify the challenges residents face and the opportunities to improve health across the county.

Two key reports emerge from this work: the Community Health Needs Assessment (CHNA) and the Community Health Improvement Plan (CHIP).

Community Health Needs Assessment (CHNA)

Understanding the challenges.

The CHNA gathers data, community input, and research to identify the most pressing health needs in Cherokee County. It provides a clear picture of the factors influencing health across our community.

Community Health Improvement Plan (CHIP)

Turning insight into action.

The CHIP builds on the findings of the CHNA to create a roadmap for improving health outcomes. It outlines the priorities, strategies, and partnerships guiding the work of Live Healthy Cherokee.
